Maintenance capability: Does it matter in the long term?

I remember working in a test department, where a particular technician, known as Mike, seemed to destroy a significant percentage of the equipment he worked on. Since he reported to me, I tried extra training, warnings, review of his workload, and finally a written reprimand. In hindsight, I should have done more root-cause analysis over Mike’s problem. Like most managers, I had my own work to complete. The equipment failure rate was just much higher when Mike was on staff. One day, he wired up a set of equipment that we needed for a time-critical test. During this task, I smelled smoke. No special root-cause analysis was needed. The last DC power supply available in the shop was smoking. Unfortunately for the instrumentation group and Mike, he got the power supply inputs and outputs reversed.
