The US Chemical Safety Board has deployed an investigative team to the scene of a fire and explosion that occurred last Thursday at the Williams olefins plant in Geismar, Louisiana.
Zachary C. Green, 29, and Scott Thrower, 47, died during the Thursday morning incident, while 77 others were injured.
More than 800 people were at the plant when the explosion happened.
The investigative team will be led by Don Holmstrom, director of the CSB's Western Regional Office. The team arrived in Louisiana over the weekend.
The CSB is an independent US federal agency charged with investigating industrial chemical accidents. The agencys board members are appointed by the president and confirmed by the Senate.
CSB investigations look into all aspects of chemical accidents, including physical causes such as equipment failure as well as inadequacies in regulations, industry standards, and safety management systems.
