I can just imagine the debate and opinions that arise regarding design mistakes. So, what is the biggest design mistake in safety instrumented systems (SISs)? Some would quickly explain that the greatest mistake is using an equipment vendor other than their favorite. "I have been using a Vionex PLC for 25 years; anything else is a big mistake." I favor new technology. And, yes, I bought a Blu Ray DVD system even before the high-definition DVD wars ended. So, it is clear that I gravitate to "new" technologies. But picking any particular programmable logic controller (PLC) is not the biggest mistake in SIS design. As long as the PLC is IEC 61508 certified, I am confident that it will be safe.
