At the conclusion of a 1983 maintenance effectiveness audit at a petrochemical plant in Cologne, Germany, the plant manager asked for a "one-second summary" of how we viewed his facility. Our one-word answer was "dependable." When the manager smiled and asked for a one-minute explanation, we said that we had just seen a wall chart in his electrical shop calling for six shots of grease to be put into each of the two bearings of electric motor MP-6064A on the first Thursday of each month. We expressed the belief that if we visited again 20 years later on the first Thursday of October, a maintenance technician would be engaged in relubricating MP-6064A. And, by the way, with six shots of grease
